International Travel Procedures

All international travel forms must be submitted at least 20 days prior to travel to clgarret@nmsu.edu. It is recommended that you do not purchase airfare until you have confirmation that your travel has been approved. 

Travel forms submitted less than 20 days prior to travel does not guarantee appropriate approval and insurance necessary for NMSU related international travel. Submitting forms early is strongly encouraged.

Employee International Travel Forms & Procedures

  1. Utilize the links below to download the form(s) you will need. 
  2. Forms are fillable in Adobe and signatures should be obtained digitally. After all signatures are obtained (with the exception of the Provost), send the completed PDFs to Cindy Garrett at clagarret@nmsu.edu. DO NOT UPLOAD travel forms into your Office of Education Abroad account. 
  3. Create an Office of Education Abroad account. Fill out the required fields and your travel itinerary. View detailed directions on how to make your account.
  4. Once information in your account is verified and the travel form(s) have been approved, you will get a follow-up email with instructions on how to download and print the approved forms. 

Do not submit any forms via Adobe Sign for signature. All International Travel forms must be submitted to Cindy Garrett at clgarret@nmsu.edu

The forms below are fillable in Adobe and are set up to be signed digitally by the filler/traveler. Please submit to the Provost Office as PDFs to clgarret@nmsu.edu

Student International Travel Forms & Procedures

The forms below are required for students traveling as part of University Related International Travel. If you have any questions, email clgarret@nmsu.edu or call 575-646-2594.

Student International Travel Authorization Form

Required for all University Related International Travel.

 

Supplemental Information

Required for travel to a country under a U.S. Department of State Travel Warning or Alert.

Waiver, Release, and Hold Harmless Form

Required for students traveling internationally for any off-campus program. 

Request for Exception To Travel Warning

NMSU Policy prohibits students to travel to a country or region under a U.S. Department of State Travel Alert or Warning unless an exception has been approved via this form.
